We now offer on-line patient registration. Our Practice Fusion electronic health record is synced to the Patient Fusion patient portal. When you schedule your initial appointment our administrative staff will request your email and we will send you an email invitation to join the Patient Fusion portal. Once registered on Patient Fusion and you have an appointment you can click on your upcoming appointment and the portal will direct you to Complete Check In. Please review and answer all questions for your first appointment. When we once again have in person sessions you can also print the New Patient Registration Form below, complete it, and bring it with you to your appointment.
